Surroundings

Clare Bridge is a reassuring place since it's designed, decorated and scaled like a family home, with both private or companion rooms.  Floor plans offer cues that help residents navigate independently.  Living rooms and dining rooms are small and intimate, so residents feel less confused as they dine and socialize with neighbors.  Outdoors there are pleasant surroundings, where safety and security are always a priority.

People

The staff at Clare Bridge is trained to understand and handle the special, unscheduled needs of the memory impaired.  More than that, are care givers get to know each resident and learn how to ease that individuals personal frustrations.  It's a family atmosphere.  And of course, our residents' families are welcome any time.


Assistance

Our resident's desire to wander is fostered by environments that are home-like and predictable.  If they carry things from their rooms, our staff simply returns them later.  Our goal is for each resident to have the right amount of personal care, support, structure, enrichment and choice, all balanced to help them feel as independent and fulfilled as possible.

Personal Service Plan

At Clare Bridge our philosophy is to provide an individualized service plan for every resident.  We believe it is your right to know exactly what you are being charged for.  Your monthly rate for quality care and services consists of three components:

The first component is the Basic Service Rate included in the basic service rate are:  Accommodations, daily meals, utilities, weekly housekeeping, laundry and linen service, life enrichment and staff 24-hours every day.  This information is also explained in the Residency Agreement.

The second component is the Personal Service Plan.  The quality care each resident will receive is determined through a comprehensive, yet easy to understand Personal Service Assessment.  The assessment will identify the personal and health care service you will receive, and allows our experienced staff to tailor a custom, Personal Service Plan around your specific needs and preferences.  We will re-assess the need for Personal Services from time to time which may result in additional charges.

The third component is the Therapeutic and Concierge Services.  These are short-term health care and preference based services that meet your unscheduled needs and wants.  You pay for the services required to maximize your independence and quality of life.

Each year close to two million Americans are diagnosed with Alzheimer's disease, Dementia or some other type of memory impairment.  While these numbers are staggering, nothing feels more devastating than learning your loved one is impacted with the disease.  Along with a diagnosis comes an uncertain future.

Symptom:  Memory Loss (recent)

  • Forgets names, phone numbers, directions
  • Needs statements repeated
  • Writes many notes
  • Hesitates in responding
  • Has difficulty in learning new information

Symptom:  Loss of Judgement and Impulse Control
Potential Behavior Signs

  • Spends large amounts of money
  • Wears inappropriate or dirty clothing; has body odor
  • Ignores rules of conduct
  • Uses coarse language, inappropriate jokes
  • Exhibits anger or irritable outbursts
  • Shows lack of concern for others

Phase 3 _______________________________________________________
  • Behavioral changes (sleep, wandering)
  • Disoriented (time and place)
  • Problems understanding and expressing self in speech and writing
  • Problems with judgment, may not recognize familiar people
  • Caregiver is isolated from friends and other family
  • May become target for individuals anger and frustration
  • May still feel sad and guilty over the loss of person they knew and loved
  • May still deny problem or refuse help
Phase 4 ______________________________________________________
  • Apathetic, unable to communicate or care for self
  • Incontinence; eventual shutting down of mind and body
  • Poor remote and recent memory
  • 24-hour care required
  • Caregiver is burned out
  • Change of role from caregiver to care manager
  • Nursing / Assisted Living / Foster Home may be best for individual, even though family will feel guilty over placement decision
