Deerwood Place is a not-for-profit assisted living community located in gorgeous, sunny North Florida.  Our elegantly appointed facility provides each of our residents assistance with their daily activities such as dressing, bathing, grooming, dining and medication supervision, if needed.  We also cater to totally independent individuals, offering assistance only when needed so that we promote the highest level of independence.  We strive to enhance self-esteem and encourage the most fulfilling lifestyle possible for our residents.

Room rates include the room, three meals per day, including therapeutic diets, 24-hour professional nursing care, linen and personal laundry, housekeeping services and recreational activities.

Respite Program

Deerwood Place offers a Respite Program with professional 24-hour a day nursing care for your loved one when you can not.  These short-term stays are available for a week, month or season.

Our respite program includes:

• Elegantly appointed, furnished private or semi-private apartments
• Three meals and snacks daily with attention to special dietary needs
• Licensed nursing staff 24-hours a day
• Assistance with bathing, dressing, and medication
• Comfortable surroundings for visiting with family and friends
• Full schedule of recreational activities or time to relax
• Housekeeping and personal laundry service

Regents Park of Jacksonville - Nursing and Rehabilitation Center, provides a full range of skilled nursing services including:

• Infusion Therapy
• Enteral Therapy
• Wound Care
• Tracheostomy Care
• Portable Diagnostics:  Laboratory, X-Ray and EKG

Our comprehensive programs meet the changing needs of both our short-term patients recovering from surgery or an acute illness, as well as the needs of our long-term residents:

Skilled Nursing Services

Skilled nursing services are suitable for patients who may have suffered from a prolonged acute or chronic illness.  Skilled nursing services may also be required by patients who may have been injured or recently undergone surgery.

General Nursing and Restorative Care

We provide general and restorative care to residents who cannot be taken care of at home due to the care giving demands of a chronic illness, the frailties of age, or physical limitations.  Our focus is on providing care and comfort while striving to enhance the joy and dignity of life.  We also offer an array of social events and recreational activities.  Our general and restorative services include:

• 24-hour comprehensive nursing care
• On-call physician coverage
• Medication administration
• Social services

Rehabilitation Services

Rehabilitation patients may require a wide-range of services depending on their individual condition and diagnosis.  Rehabilitation services include:

• Physical therapy
• Occupational therapy
• Respiratory therapy
• Speech pathology

Dietary Services

Residents are served three well-balanced, nutritious meals with many menu options and the availability of snacks.  We have registered dieticians available to accommodate any special diet needs and provide physician-ordered therapeutic diets.

Case Management

Our case management system provide sound recommendations regarding the plan of care for each patient/resident.  During care delivery, each plan is re-evaluated by all members of the team and the levels of care are adjusted according to patient/resident progress and needs.  This care planning process is an integral part of our teams ability to ensure that goals are met as quickly as possible.
